Disabling strategy specification
[here](https://github.com/paritytech/polkadot-sdk/pull/2955). (Updated
13/02/2024)

Implements:
* validator disabling for a whole era instead of just a session
* no more than 1/3 of the validators in the active set are disabled
Removes:
* `DisableStrategy` enum - now each validator committing an offence is
disabled.
* New era is not forced if too many validators are disabled.

Before this PR not all offenders were disabled. A decision was made
based on [`enum
DisableStrategy`](https://github.com/paritytech/polkadot-sdk/blob/bbb6631641f9adba30c0ee6f4d11023a424dd362/substrate/primitives/staking/src/offence.rs#L54).
Some offenders were disabled for a whole era, some just for a session,
some were not disabled at all.

This PR changes the disabling behaviour. Now a validator committing an
offense is disabled immediately till the end of the current era.

Some implementation notes:
* `OffendingValidators` in pallet session keeps all offenders (this is
not changed). However its type is changed from `Vec<(u32, bool)>` to
`Vec<u32>`. The reason is simple - each offender is getting disabled so
the bool doesn't make sense anymore.
* When a validator is disabled it is first added to
`OffendingValidators` and then to `DisabledValidators`. This is done in
[`add_offending_validator`](https://github.com/paritytech/polkadot-sdk/blob/bbb6631641f9adba30c0ee6f4d11023a424dd362/substrate/frame/staking/src/slashing.rs#L325)
from staking pallet.
* In
[`rotate_session`](https://github.com/paritytech/polkadot-sdk/blob/bdbe98297032e21a553bf191c530690b1d591405/substrate/frame/session/src/lib.rs#L623)
the `end_session` also calls
[`end_era`](https://github.com/paritytech/polkadot-sdk/blob/bbb6631641f9adba30c0ee6f4d11023a424dd362/substrate/frame/staking/src/pallet/impls.rs#L490)
when an era ends. In this case `OffendingValidators` are cleared
**(1)**.
* Then in
[`rotate_session`](https://github.com/paritytech/polkadot-sdk/blob/bdbe98297032e21a553bf191c530690b1d591405/substrate/frame/session/src/lib.rs#L623)
`DisabledValidators` are cleared **(2)**
* And finally (still in `rotate_session`) a call to
[`start_session`](https://github.com/paritytech/polkadot-sdk/blob/bbb6631641f9adba30c0ee6f4d11023a424dd362/substrate/frame/staking/src/pallet/impls.rs#L430

)
repopulates the disabled validators **(3)**.
* The reason for this complication is that session pallet knows nothing
abut eras. To overcome this on each new session the disabled list is
repopulated (points 2 and 3). Staking pallet knows when a new era starts
so with point 1 it ensures that the offenders list is cleared.

Test started failing after
https://github.com/paritytech/polkadot-sdk/commit/66051adb619d2119771920218e2de75fa037d7e8


which enabled approval coalescing, that was expected to happen because
the test required an polkadot_parachain_approval_checking_finality_lag
of 0, which can't happen with max_approval_coalesce_count greater than 1
because we always delay the approval for no_show_duration_ticks/2 in
case we can coalesce it with other approvals.


So relax a bit the restrictions, since we don't actually care that the
lags are 0, but the fact the finalities are progressing and are not
stuck.

This PR removes `AssignmentProviderConfig` and uses the corresponding
ondemand parameters from `HostConfiguration` instead. Additionally
`scheduling_lookahead` and all coretime/ondemand related parameters are
extracted in a separate struct - `SchedulerParams`.

### Summary
Some preparations for Runtime to support elastic scaling, guarded by
config node features bit `FeatureIndex::ElasticScalingMVP`. This PR
introduces a per-candidate `CoreIndex` but does it in a hacky way to
avoid changing `CandidateCommitments`, `CandidateReceipts` primitives
and networking protocols.

#### Including `CoreIndex` in `BackedCandidate`
If the `ElasticScalingMVP` feature bit is enabled then
`BackedCandidate::validator_indices` is extended by 8 bits.
The value stored in these bits represents the assumed core index for the
candidate.

It is temporary solution which works by creating a mapping from
`BackedCandidate` to `CoreIndex` by assuming the `CoreIndex` can be
discovered by checking in which validator group the validator that
signed the statement is.

## Overall idea

When approval-voting checks a candidate and is ready to advertise the
approval, defer it in a per-relay chain block until we either have
MAX_APPROVAL_COALESCE_COUNT candidates to sign or a candidate has stayed
MAX_APPROVALS_COALESCE_TICKS in the queue, in both cases we sign what
candidates we have available.

This should allow us to reduce the number of approvals messages we have
to create/send/verify. The parameters are configurable, so we should
find some values that balance:

- Security of the network: Delaying broadcasting of an approval
shouldn't but the finality at risk and to make sure that never happens
we won't delay sending a vote if we are past 2/3 from the no-show time.
- Scalability of the network: MAX_APPROVAL_COALESCE_COUNT = 1 &
MAX_APPROVALS_COALESCE_TICKS =0, is what we have now and we know from
the measurements we did on versi, it bottlenecks
approval-distribution/approval-voting when increase significantly the
number of validators and parachains
- Block storage: In case of disputes we have to import this votes on
chain and that increase the necessary storage with
MAX_APPROVAL_COALESCE_COUNT * CandidateHash per vote. Given that
disputes are not the normal way of the network functioning and we will
limit MAX_APPROVAL_COALESCE_COUNT in the single digits numbers, this
should be good enough. Alternatively, we could try to create a better
way to store this on-chain through indirection, if that's needed.

## Other fixes:
- Fixed the fact that we were sending random assignments to
non-validators, that was wrong because those won't do anything with it
and they won't gossip it either because they do not have a grid topology
set, so we would waste the random assignments.
- Added metrics to be able to debug potential no-shows and
mis-processing of approvals/assignments.

This PR will upgrade the network protocol to version 3 -> VStaging which
will later be renamed to V3. This version introduces a new kind of
assignment certificate that will be used for tranche0 assignments.
Instead of issuing/importing one tranche0 assignment per candidate,
there will be just one certificate per relay chain block per validator.
However, we will not be sending out the new assignment certificates,
yet. So everything should work exactly as before. Once the majority of
the validators have been upgraded to the new protocol version we will
enable the new certificates (starting at a specific relay chain block)
with a new client update.

`EpochDurationInBlocks`. Removal of `RococGenesisExt` prevents from
manipulating the state to change the runtime constants.

value was added: `ROCOCO_EPOCH_DURATION` (epoch duration will be set to
the value of env),
- `10,100,600` versions of rococo-runtime are built in CI and put into `polkadot-debug` docker image.

Right now, most nodes are not running it since it's opt-in using
--beefy flag. Switch to an opt-out model for test networks.

Replace --beefy flag from CLI with --no-beefy and have BEEFY
client start by default on test networks.

We consider BEEFY mature enough to run by default on all nodes
for test networks (Rococo/Wococo/Versi).

Right now, most nodes are not running it since it's opt-in using
--beefy flag. Switch to an opt-out model for test networks.

Replace --beefy flag from CLI with --no-beefy and have BEEFY
client start by default on test networks.

My guess is that `activated` used to be a SmallVec or similar, as is
