Fix outdated relevant links in readme

The Element channel is closed and the Discord channel is private for new users, so I replaced them with an invite to Stack Exchange. Not clear if the link will continue to work if the site name changes after private beta. Also replaced the link to the frontend app and removed the chat badges.
